Acute appendicitis remains one of the most common surgical pathologies requiring surgical intervention. However, recent decades have seen an increase in interest in conservative treatments for this disease, which calls into question traditional approaches to its treatment. Conservative tactics, including antibiotic therapy, can be an effective alternative to surgery, especially in cases of early stages of the disease or in patients with a high risk of complications. At the same time, the need for accurate diagnosis and assessment of the condition of the appendix becomes critically important for choosing the optimal treatment strategy. To do this, various scales and systems are used to objectively assess the degree of inflammation and determine whether conservative therapy is suitable for the patient. The article discusses the possibilities of using conservative tactics in the treatment of acute appendicitis, including the role of modern scales for assessing the condition of the appendix, such as RIPASA, Alvar.
