OPTIMIZATION OF SURGICAL TREATMENT TACTICS FOR PATIENTS WITH POLYTRAUMA OF THE ABDOMINAL ORGANS AND RETROPERITONEAL SPACE

The study is aimed at optimizing surgical tactics for treatment of the patients with polytrauma of the abdominal organs and retroperitoneal space. The purpose of this study was to identify the key factors determining treatment outcome based on a comprehensive analysis, and to propose an optimal algorithm for the surgeon’s actions. To achieve this, we have formulated the following tasks: 1) to conduct a comparative analysis of the effectiveness of various surgical tactics; 2) to identify the predictors of adverse outcomes; 3) to develop a prognostic model for treatment personalization; 4) to evaluate the results of implementing the proposed algorithm. Based on a retrospective analysis of 168 medical cases divided into two groups depending on the employed approaches, key factors determining the effectiveness of treatment were identified. It has been found that the use of minimally invasive techniques in combination with a clear algorithm of actions based on the developed prognostic model allows for significant improvement of results. Mortality in the main group decreased by 14.6% (p<0.05), the incidence of complications – by 19.2% (p<0.01). The average duration of hospital stay was reduced by 4.3±1.2 days (p<0.05). The obtained data open up prospects for the refinement of treatment protocols for this patient population taking into account the revealed predictors of adverse outcomes. They convincingly demonstrate the advantages of differentiated use of minimally invasive techniques in combination with traditional surgical approaches. The developed prognostic models and algorithms allow for personalization of the treatment strategy taking into account the patient’s individual risk profile. Further prospective studies are necessary to validate the suggested algorithm.
